2021-2022 Chromebook Agreement

I understand that I have received or will receive the following items in the event that we need to transition to Remote/Virtual instruction:

- Power Cord
By signing at the bottom of this form, I / we have read, understand and accept / agree to the terms and conditions of the Comsewogue School District:

Student / Parent Pledge for Chromebook Use

- I will use the Chromebook for educational purposes and will neither alter nor attempt to change, disable, or circumvent the management settings, content filters, or virus protection software on my Chromebook and are not permitted to install or download software that is not approved by the administration.
- I will never leave my Chromebook unattended in an unsecured or unsupervised location.
- I will not decorate (stickers, adhesives, drawings) or write on the Chromebook.
- I will be the sole user of the Chromebook and never loan it out to other individuals.
- I understand my/our responsibilities with respect to the care and maintenance of the Chromebook.
- I understand that students (grades 6-12) may take Chromebooks home in the evenings for school-related use; however, I/we understand that students must have Chromebooks in school every day.
- I will charge my Chromebook’s battery to full capacity each night.
- I will keep food and beverages away from my Chromebook since they may cause damage to the device.
- I will not disassemble any part of my Chromebook or attempt any repairs.
- I will report any issues with the Chromebook to the Technology Helpdesk in each building.
- I will protect my Chromebook by always carrying it in a secure manner to avoid damage and take all responsible precautions to prevent careless or intentional damage.
- I/we understand that the District may require me to file a police report in the event of theft or vandalism. I understand that the Chromebook I am issued is subject to inspection at any time without notice and remains the property of Comsewogue School District.
- I will follow the policies outlined in the Chromebook Handbook for Parents and Students and the District’s Computer Network and Internet Safety Policies both in and out of school.
- I agree to return the Chromebook and charger in good working condition, when required to do so.
- I agree to return the Chromebook should I graduate early, withdraw, are suspended or expelled, or terminate enrollment at Comsewogue School District on the date of termination.
- Failure to hand in your Chromebook under any of these circumstances will result in you being financially responsible for the replacement cost and/or filing of a police report for theft.
- I/we agree to pay the full repair or replacement cost of my Chromebook (or any part of) and/or charger in the event that any part becomes broken or damaged (by any means other than normal use or wear-and-tear), in accordance to the following:
     - The student/parent will be charged for the full repair and/or replacement up to full replacement cost of $300 (or a higher amount depending on cost at time of repair/replacement).
     - Power cord: Cost for a lost power cord is $50 (or a higher amount depending on cost at time of repair/replacement)
